If an employee is interested in an MD, he or she must fill-in the relevant forms, get them signed and deliver them to the helpdesk worksite (RE002).

Can I obtain an entirely new phone number?
Yes. In form 1, check the “New phone number” box in the “Select a requirement” section. When you submit the relevant forms, the MD administrator will allow you to choose from phone numbers that CZU has available from T-mobile.
Can I transfer my existing phone number from a different operator under CZU?
Yes. When submitting the form, the employee must also submit to the helpdesk a transfer code, i.e. so-called ČVOP code (notice number of the abandoned operator). Based on this code, the original operator will agree with T-mobile on the appropriate date of transferring the telephone number, which the employee will be informed about by the MD administrator.
Can I transfer my existing phone number from T-mobile with a rechargeable Twist card under CZU?
When transferring from a rechargeable Twist card, when submitting the form, you will need to submit to the helpdesk the number of the SIM card (19-digit number written directly on the SIM card).
Can I transfer my existing phone number with a subscriptions agreement with T-Mobile under CZU?
When transferring from a subscriber agreement on a person or firm under a general agreement between CZU and T-mobile, the employee first needs to know the obligations toward T-Mobile (minimum monthly payments and definite period of time) and submit the information to CZU MD administrators, which will inform him or her of the subsequent steps.
